This  review of medical and psychological literature indicates that PCOS is 
associated  with several mental health problems, including depression and 
anxiety, body  dissatisfaction and eating disorders, diminished sexual 
satisfaction, and  lowered health-related quality of life. Effective and comprehensive 
treatment of  women with PCOS must encompass careful attention to psychological  
symptomatology.

The  long-term consequences of PCOS have not been fully determined, but there 
is an  increased risk of progression to diabetes and an increase in 
cardiovascular risk  factors. The extent to which PCOS is present in postmenopausal 
women and the  degree to which it increases various risk factors in addition to 
the known risk  of the postmenopausal period are not yet known.

This  study compared the quality of life in women with PCOS vs. controls.  It 
 found that PCOS resulted in a negative impact on quality of life.  Five  
domains were evaluated - weight, hair, emotions, menstruation, and  infertility.  
The most significant impact was in the weight domain,  followed by hair and 
emotions.

The  incidence of acneform reactions was 6%. The following variables showed a 
 statistically significant effect on the percentage of patients with 
reactions:  age, with younger patients more likely to develop lesions; those treated 
with  the Nd:YAG laser type were more likely to develop lesions than those 
treated  with the alexandrite; and the Fitzpatrick skin type V showed the highest  
incidence of acneform lesions, followed by skin types II and IV. History of  
PCOS, number of prior treatments, use of aloe vera cooling gel, and the sex of 
 the patient had no apparent effect on the incidence of acneform lesions.  
Acneform reactions are relatively common after laser hair removal; however, in  
the majority of cases, the severity of the reaction was mild and lasted for a  
short duration.

Insulin  resistance is common with PCOS. It contributes to the anovulation,  
hyperandrogenism, infertility and early pregnancy loss suffered by women with  
PCOS. Current evidence supports the role of metformin in the treatment of  
anovulation in PCOS. In addition, novel evidence suggests that insulin  
sensitizers may also play a role in reducing the risk of early pregnancy loss.  The 
insulin-sensitizing agents available commercially include metformin,  
rosiglitazone and pioglitazone. Metformin is the agent that has been most  frequently 
studied in PCOS, and has the most favorable pregnancy safety profile.  The study 
concludes that there is strong evidence supporting the use of  metformin as a 
primary ovulatory agent in women with PCOS.

Metformin  was compared to the oral contraceptive pill (OCP) in treating 
PCOS.  The  data demonstrated no evidence of a difference in effect between 
metformin and  the OCP on hirsutism, acne or development of type 2 diabetes.  
Metformin,  in comparison with the OCP, was less effective in improving menstrual 
pattern  and in reducing the serum total testosterone level weighted mean 
difference but  more effective in reducing fasting insulin and not increasing 
fasting  triglyceride levels. The data demonstrated no evidence of a difference in 
effect  between the two therapies on reducing fasting glucose or total 
cholesterol  levels and severe adverse events.
