Hyperandrogenism and infertility represent the major complaints of PCOS in adult fertile age. In addition, obesity and metabolic syndrome may affect more than half these women. Later in life, it becomes clear that the association of obesity, particularly abdominal obesity, and PCOS renders affected women more susceptible to develop type 2 diabetes. Little is known about ovarian morphology and androgen production in women with PCOS after menopause. Some studies found that morphological ultrasonographic features consistent with polycystic ovaries are very common in postmenopausal women, and that these features are associated with higher than normal testosterone levels and metabolic alterations. ************************************** See what's free at http://www.aol.com. -------------- next part -------------- An HTML attachment was scrubbed... The study concluded that PSA appears to be a promising marker of endogenous androgen excess in females suffering from PCOS. ************************************** See what's free at http://www.aol.com. The PCOS groups were: 1) high androgens and irregular periods 2) high androgens and normal periods 3) normal androgen and irregular periods In regards to insulin sensitivity and metabolic syndrome, women in group 3 couldn't be distinguished from controls and were markedly less insulin resistant than women in group 1. For group 1, the LH levels were intermediate between the controls and group 3. The study concluded women with PCOS who have normal androgens but have irregular periods are metabolically similar to control women with significantly fewer metabolic features than PCOS women who have high androgens. However, higher than normal LH and lower sex hormone-binding globulin (SHBG) concentrations in group 3 support the view that they form part of the spectrum of PCOS. ************************************** See what's free at http://www.aol.com. -------------- next part -------------- An HTML attachment was scrubbed... Carbamazepine is sold under the brand names Tegretol, Tegretol XR and Equetro. The aim of the study was to compare the effects of carbamazepine and oxcarbazepine on the reproductive endocrine function in women with epilepsy. The study concluded carbamazepine and oxcarbazepine have different effects on the reproductive endocrine function. Although both drugs were associated with low serum testosterone concentrations and low free androgen indexes , only oxcarbazepine was associated with a high frequency of elevated levels of androstendione and DHEAS and with an increased prevalence of polycystic ovaries. These findings suggest that oxcarbazepine may be disadvantageous for women with epilepsy and hyperandrogenism, whereas carbamazepine may be beneficial for these women. ************************************** See what's free at http://www.aol.com. A cross section of postmenopausal women who had BMIs between 21.5 and 27.5, were between 65.9 and 81.7 years old and weren't using estrogen were studied. Women who were included in the PCOS group had at least 3 of the following: (1) recalled history of irregular menses, (2) symptomatic premenopausal hyperandrogenism or biochemical evidence of current biochemical hyperandrogenism, (3) history of infertility or miscarriage, (4) central obesity, or (5) insulin resistance. PCOS was found in 9.3% of the women studied and 5.8% of the women without diabetes. The study concluded that PCOS increases the risk of atherosclerotic CVD after menopause, especially in non-diabetic postmenopausal women with intact ovaries. ************************************** See what's free at http://www.aol.com. The objective of this study was to evaluate the diagnostic utility of fasting tests compared with OGTT tests in women with PCOS. 114 women with PCOS and 29 normal women were given a 2 hour OGTT. Fasting insulin and glucose were measured at the time of the test. A significant correlation between the fasting and OGTT test results was found. Normal fasting tests with abnormal OGTT results was only found in 9 women. IR was not found in any woman whose fasting insulin was less than 9.9 Ul/ml . All women with fasting insulin over 18.4 Ul/ml were found to be IR. The study concluded that fasting tests are useful in the diagnostic of IR in PCOS patients. OGTT is necessary when the fasting insulin levels range between 9.9 and 18.4 Ul/ml. ************************************** See what's free at http://www.aol.com.
