Acute effects of metformin therapy include improvement of insulin resistance

This study compared PCOS patients with (IR) and without (NIR) insulin
resistance to controls. All were put on 850 mg of metformin per day for a week.
Several blood tests were statistically significantly higher in patients with
PCOS compared to controls. These same blood tests were similar among IR and
NIR patients with PCOS. One week of metformin therapy did not alter the blood
test levels. However, there was a statistically significant increase in
glucose/insulin ratios, and a significant decrease in the number of follicles
after 1 week of metformin therapy. The study concluded that levels of these
blood tests are significantly increased in patients with PCOS compared to
controls, but are not associated with insulin resistance. Low-dose metformin
therapy improves IR and polycystic ovary morphology, even though the blood test
levels remain unchanged.
